For those who have reserved admission

  • If you do not tell the hospitalization time at the time of admission reservation, the Patient Support Center will contact you by phone the day before hospitalization (weekday afternoon).
  • If you do not have a fixed hospitalization date, the hospital will contact you as soon as the hospitalization date is decided. Please note that in some cases, you may be contacted the day before hospitalization.
  • Depending on the circumstances of the hospital, you may not be able to enter the hospital on the scheduled date of admission. In that case, we will contact you by telephone on the day you can be hospitalized.
  • The decision to be hospitalized will be the one who has been instructed by the doctor in charge as a result of the outpatient clinic.
    ※If you are unable to be hospitalized due to circumstances, please contact the outpatient department as soon as possible.
    ※If you cannot come to the hospital at the scheduled time due to circumstances, please contact the Patient Support Center.

About hospitalization procedure (on the day of hospitalization)

Please perform hospitalization procedure in "hospitalization reception desk (No. 9)" on the first floor. In the case of night or on holidays, please go to the "emergency office" on the first floor.

What is required for the procedure

  1. Certificate of hospitalization
  2. Explanation and consent form regarding non-insurance charges
  3. Private room entry application form (If you want to use or wish to pay private rooms, please submit it. If you have moved from a large room to a private room, please submit it at a later date. )
  4. My number card (or health insurance card) and various medical cards (if you have one)
  5. Medical examination ticket (ID card)
  6. Seal
  7. Others
    If you fall under any of the following, please be sure to report to the “Inpatient Reception (No. 9)” on the 1st floor.

(b) Application of work injury 
(b) Traffic accident
(a) Third-party acts (such as treatment of injuries received from others)
(d) Have been hospitalized at another medical institution within 3 months (Please bring your discharge certificate)

What you need to prepare

  1. Pajamas or sleeping clothes (front opening front) and underwear
  2. Towel and bath towel
  3. Body soap, shampoo rinse
  4. Drinking cups (plastic cups) and toiletries
  5. Indoor shoes (no slippers and with heels)
  6. Tissue paper
  7. Writing utensils
  8. Earphones (for TV)
  9. Eyeglasses, etc. (daily used)
  10. Drugs currently taking, "medicine notebook" or "medication of medicine"
  11. Those designated by each outpatient department (consent form, etc.)
