2. About consultation

5. Others

(1) About my number card (or insurance card)

  • Please be sure to register once a month at the My Number Card authentication terminal next to the first consultation reception.
  • If you can not confirm for a long time (3 months or more), you will have to pay for it.
  • If your insurance card information changes, be sure to submit it to the reception.

(2) About medicine notebook

  • In our hospital, we ask that you bring your medicine notebook at the time of consultation or hospitalization so that patients can receive safe treatment safely.
  • Checking the current status of oral dose in your medicine notebook is very important information in deciding your future treatment policy.
  • Especially before the test or before surgery, it is necessary to check the medicine you are currently taking, and depending on the type of medicine, you may be asked to cancel it in advance.

(3) About consultation of minors

  • In this hospital, if a minor under the age of 18 is outpatient clinic, in principle, we ask that a guardian or legal agent be accompanied.

(4) When address, name, etc. change
