1． Those who need the first consultation procedure

Those who are visiting our hospital for the first time.

  1. Fill out the medical treatment application form and submit it to the first consultation reception together with your health insurance card (public medical certificate, etc.).
  2. When the first consultation procedure is completed, we will return your health insurance card and give you a consultation ticket and medical questionnaire.
  3. Please fill out the questionnaire form and submit it to the counter of the department where you are going to consult.

2． Those who do not have a reservation

Those who have been consulted at our hospital at least once

  1. Put the consultation ticket in the reception machine, complete the consultation procedure, and receive the reception slip.
  2. Please submit the reception slip and health insurance card to the window of the department you are going to consult.
    ※For ophthalmology, psychiatry / psychiatric oncology, and neurosurgery, the reception method differs, so please consult with the first consultation reception desk.

3． Those who have a reservation

  1. Put the consultation ticket in the reception machine, complete the consultation procedure, and receive the reception slip.
  2. Please submit the application form to the window of the department you are going to consult.

4． Those who have a letter of introduction

Those who are visiting our hospital for the first time.
Fill out the medical treatment application form and submit your health insurance card (public medical certificate, etc.) and a letter of introduction to the first consultation reception.

Those who have been consulted at our hospital at least once
Please send a letter of introduction to the letter of introduction to the reception desk.

※　When you come to our hospital, please be sure to bring your consultation ticket. Please note that if you lose or forget, it will be reissued for a fee (110 yen).

5． Others

If you are a first-time consultation at our hospital or have been consulted, but if this medical treatment is judged to be the first consultation, we will collect a separate fee.
